In your example you put the fault on the Captain of the ship. What you don't say is what happened to the two persons who fell asleep, their immediate supervisor and the chain of command up to the Captain level. Did the two sailors get "blamed" or accused of a service offence?

I am sure that the Captain's accountability and responsibility included ensuring that this incident did not repeat itself and that everyone involved got the necessary "adjustment".

All members of the forces should take responsibility for their individual conduct - regardless of rank and position.
